Respiratory diseases in Kazakhstan, as well as all over the world, are among the most common ones. Children, adults and people of the older generation frequently fall ill with them. In many cases, diseases cause loss of ability to work, reduce quality of life, and often lead to a fatal outcome. Respiratory diseases are a serious economic problem because of the amount of lost work time. To improve care for patients with respiratory pathology, the decision of the management of the Medical Center and Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan on the basis of the pulmonology department Respiratory Medicine Center was established. The center functionally united specialists of different areas, providing care for respiratory pathology.

Medical care for patients with bronchopulmonary pathology in the Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan is provided by pulmonologists of inpatient and outpatient departments with active support of endoscopic service, department of CT and MRI diagnosis, X-ray department, departments of functional diagnosis and pathomorphology, clinical diagnostic and microbiological laboratories, department of physiotherapy and rehabilitation. In the process of specialists performance contacts and partner relations were established with leading specialists of regions of the Republic of Kazakhstan, Ministry of Healthcare of the Republic of Kazakhstan. The service was supervised by chief therapist, pulmonologist, Dr. R.Bakenova.

  1. Assistance in all directions of respiratory medicine is rendered to the assigned contingent and patients from the regions of Kazakhstan (on a fee basis): obstructive, infectious, allergic, interstitial diseases, including idiopathic, in the systemic diseases of connective tissue, professional etiology, from drug, radiation exposure, bronchopulmonary dysplasia, post-COVID lesions of bronchopulmonary system, etc.
  2. Advisory, diagnostic and treatment assistance is provided to patients with interstitial lung diseases to the fixed contingent and patients from the regions of Kazakhstan (only in Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan a full range of treatment and diagnostic services in accordance with clinical protocol, international recommendations).
  3. Transbronchial biopsy of the lung and intrathoracic lymph nodes under CT and endoscopic ultrasound monitoring has been introduced and is actively performed (EBUS - in Kazakhstan only in Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan).
  4. Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan is the only clinical center in Kazakhstan and the Central Asian region, where the study of pulmonary diffusion capacity with carbon monoxide by single inhalation (DLco study - in Kazakhstan only in the Medical Center Hospital) is introduced and being actively conducted.
  5. The body plethysmography (BPM) method, which has no analogues in the Republic of Kazakhstan, has been introduced in the Medical Center Hospital for the first time in Kazakhstan, which allows to study respiratory mechanics disorders in depth, as well as lung volumes and capacities.
  6. Due to the high level of trained specialists, availability of high-tech, expert class equipment, there are opportunities to identify rare (orphan) interstitial lung diseases - lymphangioleiomyomatosis, tuberous sclerosis, alveolar proteinosis, broncho-pulmonary dysplasia, etc.
  7. Respiratory tuberculosis, non-tuberculous mycobacterioses, and oncological diseases of the bronchopulmonary system are detected.
  8. A sleep laboratory is actively functioning, where serious sleep breathing disorders are identified with respiratory monitoring and polysomnography with the selection of individual programs of hardware treatment.

The high level of training of Respiratory Medicine Center specialists, specialized and expert equipment were the basis for organization and implementation of consultative and diagnostic assistance in the regions of the Republic of Kazakhstan. Together with the chief specialist of the Ministry of Healthcare of the Republic of Kazakhstan, National Society of Respiratory Medicine (NSRM) the analysis of pulmonological service of the Republic of Kazakhstan is carried out with the recommendations on its development.

The Republican Multidisciplinary Pulmonary Commission (RMPC) has been established to select patients with progressive pulmonary fibrosis (PPF) for expensive antifibrotic therapy. Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan is a clinical training site for residents of NC JSC "Astana Medical University", NC JSC "Karaganda Medical University", Nazarbayev University (UMC). Within the framework of its activity the specialists of the Center of Respiratory Medicine conduct scientific and clinical researches with analysis and publication of results, presentations, reports at international and republican forums.

As part of assistance to specialists in the regions of Kazakhstan, specialists of the Respiratory Medicine Center of the Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan conduct online and off-site master classes, webinars on respiratory medicine, clinical consultations of complex cases of respiratory diseases are organized.

The 1st National Congress of Respiratory Medicine was organized and held in November 2022 with the participation of specialists from the BMC Respiratory Medicine Center (8 reports from the Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan were presented, a symposium on ILD was held by specialists from the Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan with the participation of Prof. Prosch H (Austria), a Scientific and Practical Conference with international participation "Interdisciplinary problems of allergology and immunology" was organized and held at the Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan. Pulmonologists and allergologists from the Medical Center Hospital presented 6 reports and held 3 seminars. As part of the activities of the Respiratory Medicine Center of the Medical Center Hospital, partnerships are being established with leading respiratory medicine specialists from near and far abroad in order to prepare joint scientific and clinical projects. In order to improve diagnosis and treatment of patients with interstitial lung diseases (ILD), which is the most difficult and understudied problem of respiratory medicine, to improve the quality of ILD statistics, to improve the ability to predict the situation of ILD, to plan measures to improve patient care, to introduce new technologies for this pathology, a register of interstitial lung diseases is developed by the Center specialists together with leading specialists of Kazakhstan.

At this stage, within the framework of partnership relations, the preparation for 2nd National Congress on Respiratory Medicine (November 9-11, 2023) together with National Society of Respiratory Medicine, Republican Interdisciplinary Pulmonary Commission, with participation of chief specialist of the Ministry of Healthcare of the Republic of Kazakhstan in accordance with the plan of scientific-practical conferences of Ministry of Healthcare of the Republic of Kazakhstan is conducted.

By results of scientific clinical researches, conducted in the Center in the sphere of pulmonology, 2 master dissertations are protected by specialists of pulmonology department of the Medical Center Hospital, specialists of pulmonology department of Medical Center Hospital of the President's Affairs Administration of the Republic of Kazakhstan are preparing for conduction of scientific clinical researches by PhD doctoral program. The specialists of Respiratory Medicine Center have developed and passed expert assessment of Clinical Protocol of diagnostics and treatment "Idiopathic pulmonary fibrosis. Progressive pulmonary fibrosis".

Currently, specialists of the Center of Respiratory Medicine at the Medical Center Hospital are planning to introduce surgical lung biopsy (video-assisted lung biopsy), endobronchial lung cryobiopsy, thoracic service organization, etc. into their activities. The Center also plans to actively cooperate with respiratory societies of Kazakhstan, foreign and neighboring countries (ERS, ATS, RRO, NORM, EARO, etc.), leading university clinics in Europe to exchange experience, conduct master classes, international conferences, search and introduction of new technologies in the respiratory medicine.
